
CPHC & Community Partners' Centre of Excellence
CPHC and Community Partners' are building a Health and Wellness
Centre of Excellence for residents of all ages.
This Centre, to be located in Brockville, will be built in two phases. The Centre will serve residents of Brockville, Leeds & Grenville and those visiting Brockville or willing to travel to Brockville will be welcome. Construction completion date is March 15, 2011.
The Centre will ensure improved accessibility for multiple services; the building will adhere to all requirements for those who have disabilities/challenges of any sort. Barriers will be reduced as construction will adhere to all codes in relation to disabilities; there will be ample parking. The project will bring Health, Community & Social Services as well as other complementary services together in one building. Currently services are scattered across many sites making it difficult for residents to access in a timely manner. Residents will have the opportunity to participate in, and continue to contribute to their community. Optimal health and wellness ensures residents can continue to contribute economically.
PHASE I ($9.4 MILLION): Project commencing Fall of 2009 will include the following:
- CPHC Brockville Community Family Health Team currently has 3 sites in Brockville. This team, providing interdisciplinary access to primary health care through Doctors, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians will relocate to the Centre in Phase I. Arthritic Clinics (with physiotherapists) and over 20 Wellness Clinics (inclusive of Chronic Disease Management services) will also be offered through the Centre. - Capital dollars approved by the Ministry of Health & Long Term Care.
- CPHC Community Support Services including: Diners Clubs, Meals on Wheels, Friendly Visiting, Telephone Reassurance, Home Help/Home Maintenance, Foot Care, Adult Day, Caregiver Support (i.e., respite, education and counseling), Essential Transportation (i.e., to medical appointments including medical specialists in Kingston or Ottawa and other essential services).
- Lifeline serving Kingston, Lanark, Leeds & Grenville.
- CPHC Caregiver Support Centre includes Adult Day Program, Guest (Respite) Centre.
- CPHC Seniors' Exercise Program - volunteer instuctors are certified locally by a CPHC staff instructor, through the University of Western Ontario (Canadian Centre for Activity and Aging - CCAA). This program currently has over 40 volunteer instructors and over 400 participants across 18 sites throughout Lanark, Leeds and Grenville Counties.
- CPHC Diabetes Education Centre provides service to residents who are pre-diabetic and those living with diabetes.
- Auditorium for Social, Educational and Recreational Events.
- CPHC Outdoor Wellness Park for seniors and those physically or otherwise challenged (similar to model in British Columbia). - Capital Funding will be raised locally.
PHASE II: timing for this phase is indefinite, but when approved, this phase will consist of up to 80 senior units - both affordable housing and supportive housing.
