The CPHC Gananoque Community Family Health Team is located at 338 Herbert Street, Gananoque. Patients of the CPHC Gananoque Family Health Team have access to Family Physicians, Nurse Practioner, Registered Nurses, Registered Dietitian, Social Worker, Psychiatrist and other health care specialists.

Appointments are required. Morning clinics are available Monday through Friday from 8:00 – 10:30 a.m. for registered patients only. No appointment necessary.

As of January 11, 2010 CPHC Gananoque Community Family Health Team will commence its participation in a 2-year pilot project introducing a Physician Assistant (PA) to the team.
