The CPHC Community Family Health Teams are comprised of physicians, nurse practitioners, registered nurses, dietitian, psychiatrist and other health care providers, providing a full range of health care services to individuals of all ages, families and the community. These health teams:

Increase access to healthcare services

Provide services through a multi-disciplinary team

Ensure a focus on integration and coordination of services through partnerships and implementation of an IT strategy

Provide exceptional health promotion, disease prevention, chronic disease management and comprehensive primary health care services responding to local population needs
A visit can include:

Annual health assessment
-
Lifestyle modification counseling and support to individuals and groups

Routine prenatal/post-partum care, baby wellness and pediatric exams
-
Monitoring and treatment of chronic illnesses such as: diabetes, asthma, COPD, arthritis, and high blood pressure

Orders for diagnostic tests such as blood work, x-rays, ultrasounds and mammograms

Prescriptions for medications

Treatment and diagnosis of common health concerns such as ear and bladder infections

Wellness care including health screening such as pap smears, monitoring infant growth and development, well woman care and immunizations

Supportive care through illness
-
Treatments and/or procedures such as suturing of wounds

Referrals to family physicians, specialists and other health professionals

Working with YOU to develop strategies to manage your health through specialized clinics such as PAP screening, blood pressure and diabetes management
