Adult Day Services
This community-based respite service is designed to meet the needs of physically disabled and/or cognitively impaired adults. Social and recreational needs are met within supervised, safe and secure environments at various locations throughout Leeds and Grenville. There is a fee for this service. Subsidization may be available for those who cannot afford the full fee.
Caregiver Support - In-Home Respite Service
Respite caregivers provide support service in a client's home to provide relief for family or friends from their care giving role and to help relieve stress placed on family relationships. Stimulation appropriate to the clients' interests, activation and socialization are key components. Service may include homemaking, personal care, attendant care, monitoring, and supervision. There is no fee for this service. Client Paid Respite is also available.
Caregiver Support and Counseling Service
Provides therapeutic counseling to caregivers and family members who have severe stress that impedes their ability to provide care/support for the client. This service is time limited, goal-directed and targeted to meet the caregiver's social and emotional needs. The service is provided to caregivers and family members either in group or by a professionally trained individual and/or one who has demonstrated knowledge and expertise in the client's area of need. There is no fee for this service.
Caregiver Information, Education and Training
Assists the caregiver and/or family members by providing emotional support, information on the disease process and available resources, assistance in problem solving and coping skills. The service is provided in group or individual sessions. There is no fee for this service.
Music and Memories Luncheons
Specially trained staff provide support for Community Support Services 'Music and Memories' Diners Clubs across Lanark, Leeds and Grenville. There is a fee for this service. Subsidization may be available for those who cannot afford the full fee.

Client Intervention
CPHC responds to calls for service and will make a home visit to assess the needs of clients and will provide services or make referrals to other agencies. There is no fee for this service.
Transportation Service
CPHC volunteers drive clients to essential services such as medical appointments, Adult Day Programs, grocery shopping etc. Rides are available for local and long distance destinations. There is a fee for this service. Subsidization may be available for those who cannot afford the full fee.
Diners Clubs
Join us, on pre-arranged days as we meet for a nutritious meal, social activity and friendly companionship. Rides are available. There is a fee for this service. Subsidization may be available for those who cannot afford the full fee.
Friendly Visiting
CPHC volunteers are carefully matched with clients to provide companionship and support on a regular basis. Whatever the activity... your volunteer and you decide together! There is no fee for this service.
DUET: Enhanced Friendly Visiting/Companion Care
DUET: Enhanced Friendly Visiting Companion Care will provide an alternative to traditional respite for the caregiver and their loved ones. Seniors will act as volunteers to provide companionship to the caregiver's loved one while the caregiver remains in the comforts of their home. This enables caregivers to pursue worry-free activities such as reading, sewing or resting in their own home while getting a 'break' from their caregiver role. There is no fee for this service.

Telephone Reassurance
CPHC volunteers telephone clients daily, or as requested, to provide a little social contact and to see how they are doing. There is no fee for this service.
Foot Care Clinics
Regular foot checkups can be important in the early diagnosis of many illnesses. Proper foot care keeps your feet healthy, problem-free and helps to ensure you stay active and independent. Foot care clinics are provided every 4 to 6 weeks by specially trained nursing professionals, on an appointment basis, at various locations, including Brockville, Cardinal, Athens, Westport and Prescott. There is a fee for this service.
Meals On Wheels
CPHC volunteers deliver hot and delicious meals to clients who find it difficult to prepare their own, or who are convalescing.
A wide variety of nutritionally balanced, flavourful frozen meals are also available; they can simply be reheated in a regular oven or microwave. These meals do not contain additives/preservatives.
You may choose to combine hot and frozen meals. Special dietary needs can also be accommodated. There is a fee for Meals On Wheels. Subsidization may be available for those who cannot afford the full fee.
Home Help/Homemaking
CPHC maintains a registry of screened workers who will help clients maintain a clean and safe environment inside their home - dusting, vacuuming, other light housekeeping, laundry, shopping and meal preparation. The focus is on residents who can neither undertake nor arrange for these services themselves. There is a fee for this service. Subsidization may be available for those who cannot afford the full fee.
Home Maintenance and Repair
CPHC maintains a registry of screened workers who will help clients with yard maintenance, window cleaning, snow removal, raking, etc. As well, CPHC will help clients arrange for one-time jobs such as plumbing, roofing and other repairs. There is a fee for this service. Subsidization may be available for those who cannot afford the full fee.
