SAIL Transfer to CPHC

Community & Primary Health Care (CPHC) welcomed the transfer of Services to Assist Independent Living (SAIL) effective April 1, 2011.

Through the support of the South East Local Health Integration Network (SE LHIN), SAIL, CPHC and the communities-at-large, SAIL Services transferred to CPHC on April 1, 2011. We expect clients, caregivers, staff, volunteers and the community will find this transition smooth and seamless.

CPHC is thrilled that Linda Brooks (previously President of the SAIL Board of Directors and Kathleen Burtch - previously Vice President of SAIL Board of Directors) have joined the CPHC Board of Directors effective April 1, 2011.

With the transfer of SAIL programs, individuals and caregivers will continue to receive services they currently access from SAIL, including Meals on Wheels, Transportation, Congregate Dining, Friendly Visiting, Security and Reassurance, Footcare, the Walking Program and many more. The CPHC Gananoque site is located at 740 King Street West (same site as previous SAIL site). Staff have transferred to CPHC; volunteers have been encouraged to continue to serve the SAIL clients.

SAIL services are complimentary to CPHC's programs; the transfer will result in enhanced services and reduce confusion that may exist in relation to CPHC & SAIL service jurisdictions. Currently, CPHC's service areas cover Lanark, Leeds, Grenville (and Kingston for Lifeline).

CPHC is a not-for-profit organization; the generous contributions of Donors have greatly assisted in ensuring continuous and exceptional service. Donors are encouraged to continue to be an integral part of the combined agency as funds raised locally will remain in their respective communities.

The CPHC Board of Directors, management, staff and volunteers welcome the SAIL staff, volunteers and clients. We look forward to continuing to work together to bring vital programs to the communities we serve.
