
CPHC & Community Partners - Centre of Excellence
CPHC and Community Partners will build a Health and Wellness
Centre of Excellence for residents of all ages.
This Centre will be built on Parkedale Avenue in Brockville and will serve residents of Brockville, Leeds and Grenville and those visiting Brockville or willing to travel to Brockville. The date for completion is December 2012.
The Centre will improve accessibility with ample parking; barriers will be reduced as construction will adhere to all codes in relation to disabilities. The project will bring Health, Community, Social and other complementary services together. Currently, local CPHC services are across seven (7) Brockville sites, making it difficult for residents to access in a timely manner. These seven (7) sites plus up to four (4) other community agencies will transfer to the new site. Residents will have the opportunity to participate in and continue to contribute to their community; enjoying optimal health and wellness, thus ensuring an enhanced quality of life.
PHASE I: MINISTRY OF HEALTH & LONG-TERM CARE - CAPITAL APPROVED - $3,697,010
- CPHC Community Family Health Team Brockville Site - currently has 3 sites in Brockville - interdisciplinary access to primary health care through Doctors, Nurse Practitioners, Registered Nurses, Social Workers, Dietitians and Clinics such as Arthritic Clinics (physiotherapists), Congestive Heart Failure Clinics and Wellness Clinics inclusive of Chronic Disease Self Management Services
PHASE II: ENABLING ACCESSIBILITY FUND - HUMAN RESOURCES AND SKILLS DEVELOPMENT CANADA - MID-SIZED CAPITAL GRANT - $3,000,000
- CPHC Community Support Services include Diners Clubs, Meals on Wheels, Friendly Visiting, Lifeline Telephone Reassurance, Home Help, Home Maintenance, Foot Care, Adult Day, Caregiver Support, including respite, education and counseling, Essential Transportation to medical and essential services, including medial specialist in Ottawa or Kingston (i.e. those needing bypass surgery, etc.)
- CPHC Caregiver Support Centre includes Adult Day Program, Guest (Respite) Centre.
- CPHC Lifeline serving Kingston, Lanark, Leeds & Grenville.
- CPHC Outdoor Wellness Park for seniors and those physically or otherwise challenged (similar to model in British Columbia).
- CPHC Heart Wise Seniors Exercise Program - volunteer instuctors are certified locally by a CPHC staff instructor, through the University of Western Ontario (Canadian Centre for Activity and Aging - CCAA). Currently, this program has over 40 volunteer instructors and over 550 participants across 20 sites in Lanark, Leeds and Grenville Counties.
- CPHC Diabetes Education Centre provides service to residents who are pre-diabetic and those living with diabetes.
- Auditorium for social, educational and recreational Events.

- Survey (retrace property boundaries; tie in all surface features including roads, sidewalks, buildings, creek, culverts, gravel, asphalt and above ground services; prepare a large contoured geographic plan of the site suitable for design work) - January 11, 2011 - done
- To Tender - September 2011
- Capital Committee Campaign - $2.9 million - Commence summer 2011
- Shovel in ground - Late Fall 2011
- Completion and Opening - December 2012
